- They need easier access to technology and information.
- Better access to research. Many of the decision makers in the health sector don't realize the importance of research and push Ophthalmologists to increase the number of patients seen. Not enough time or support is provided to Ophthalmologists for research.
- Faster acceptance and adoption of new technology.

Within the United States the biggest concern will focus on the onslaught of the baby boomer generation's demand for care. Having a full waiting room will no longer signal a productive high quality practice it will signal poor management of patient care

1. Coping with the furious pace of government 'modernisation' of the National Health Service in the UK
2. Keeping up to date, not only with the exponential growth of knowledge, but having to 'revalidate' to demonstrate competence
3. Staying in work and financial indepencence. Medical unemployment is more likely than ever before. The UK will be producing 7000 medical students annually by 2010. The NHS is being privatised and competition is being introduced. Meanwhile the world is desperately short of ophthalmologists.
4. Maintaining skill levels in the face of increasing sub-specialisation in ophthalmology

1. Good, sound knowledge and appropriate clinical and surgical skills to deal with day to day problems.
2. Access to appropriate and affordable technology for the above.
3. A good understanding of the community in which he works including their needs.

Dear mister President Bruce Spajvej!
We are grateful for Your letter and attention to problems of blindness in our country Tadjikistan. I bring apologies, for possible, weak translation which is made by a computer.
In reply to your letter I inform the following: after civil war in To our country which is in a transition period to To democratic secular society, medical and ophthalmologic The equipment morally also is physically obsolete, not replenishing with 1960-1970 Years of the last century. We,
ophthalmologists, work on naked enthusiasm, helping our blind and visually impaired. We do not have modern elementary technologies in ophthalmology, such as tonometers, удьтразвуковые devices, microscopes, Electric офтальмо and biomicroscopes, it is especial in regional cities and Areas. The microsurgical toolkit also is obsolete. Ours Ophthalmologists also have no opportunity to fill up the knowledge for Boundary as cannot pay such training on rates Ophthalmology and the stay in other country and payment of travel Roads.
As to personally me, as unique professor-The ophthalmologist in the country, the President of the Republican society Ophthalmologists I for last 15 years could not visit any The international forum on ophthalmology to bring in the country the To colleagues the new information.
We have no any financial supports Neither on the part of Ministry of Health, nor on the part of others the organizations. Methodical maintenance with manuals of faculties Ophthalmology of medical university wishes Updating as too it is very obsolete. A society blind children's and adult population
also is in a pitiable condition. In connection with above-stated, we shall be grateful to reception of the help in its(her) any kind. For the blessing our patient with a pathology of eyes: to preparation of doctors, educational, Technical (computers, diskettes, picture albums, slides, presentations
ит.д.) grants(manuals), the ophthalmologic equipment and toolkit, Improvements of a social status blind and visually impaired, etc. I did not write to you about our troubles because did not hope on earlier any help. Now, having read your letter, the hope has appeared and therefore I write
frankly to you about our situation. Beforehand I thank, with hope to the aid.
The president of the Republican Society of Ophthalmologists of Republic Tadjikistan, managing faculty of ophthalmology medical University Professor

Financial security is paramount to our members in order to maintain good office staff, OR nurses, educational programs for future associates, and access to latest techology or creation of that technology.
As leaders of these organizations, we simply can't lose sight of adequate funding for the mission - which in our case is primarily education as well as patient and surgeon advocacy. As potential patients, none of us want to see the erosion of our standard of health care.

The greatest needs of individual ophthalmologists in my country as well as neighboring countries in South East Asia would be new knowledge and skills. There is not enough efforts to dissipate new knowledge. There are not enough training opportunities for the ophthalmologists to acquire modern skills. The main reason for not able to update is lack of inadequate training and knowledge rather than financial security.

I think a more flexible exchange program is needed for young ophthalmologists.
an opposite question will be what the community need from the ophthalmologists, I think that ethics should be considered as a major in ohthalmic education.

I think the greatest needs for individual ophthalmologists in the UK over the next 5-10 years will be adjusting to the changing reorganisation of the Health Service delivery in the UK.
Increasingly it is becoming obvious that the old system of our NHS is not delivering a service which is sufficiently accountable to satisfy politicians and the public. Changes in what the government wants will mean changes in what the individual ophthalmologist is asked to deliver and this will be what will govern the greatest needs for an individual ophthalmologist.
In practical terms it will mean keeping up to date, learning new skills and being flexible about the working place, hours and the type of service he delivers i.e. the balance between government funded and practice.

Appropriate skills, equipments and good remuneration are basic needs of an ophthalmologists in a developing country. Beyond this is a need for subspecialization which is currently hampered by registration for hands-on in most countries with good training programs. We needs thoughts on how to make this possible.
