Documents Required to Apply for the ICO Three-Month Fellowship

Scanned copy of passport
Copy of ICO Exams Certificate(s). The ICO recommends that candidate pass one or more of the ICO Exams.
Scanned copy of Specialist Exam Certificate, Diploma of National Board, Master of Surgery, or equivalent
Present Chair’s Recommendation & Residency Confirmation Letter (DOC file: 80 KB)
Previous Chair’s Recommendation (DOC file: 80 KB)
ICO Fellowships Program Host Notice of Acceptance-Three Months (DOC file: 502 KB) *Fellows are required to complete the Candidate and Fellowship sections of the form. Hosts will complete the Budget section and sign/date as appropriate.

If appropriate, you may be required to upload certified translations in English.

**Note: Uploaded file types are limited to: jpg, pdf, doc, docx, and png. There is a 20MB size limit for all uploads. Zip files are not allowed.

If you are awarded a fellowship, the Fellowships Executive will request a copy of your visa, verification of your health coverage, and verification of your liability insurance coverage.

Letter of Recommendation (DOC file: 80 KB) of department chair documenting your training and surgical experience, and potential benefits of fellowship to home country. This letter may be from a previous chair and is different from Present Chair's Recommendation & Confirmation Letter.
Letter of Confirmation of Post-Fellowship Position (DOC file: 80 KB)
